Is it true and correct to say that even the prophet would not enter paradise if Allah questioned every action? People will enter by the grace of Allah.

It is true that everyone who enters paradise will do so by the grace and mercy of Allah. However it is the belief of the Ahlus Sunnah wal Jama’ah that the Prophets are sinless. Allamah Taftazani mentions that a saint can never reach the status of a Nabi and among one of the stated reasons for this is the fact that Prophets are Masoom (sinless) (Sharhul Aqaid)
Therefore on that basis to say even the prophet would not enter paradise if Allah questioned every action is incorrect and the person who is of this opinion should make sincere repentance.
