Question:

If someone has done zina, then repented will full heart, does he have to tell anything about this to his wife?

Answer:

As a general rule, one should not reveal their sins to anyone, not even their close family members.

However, in a case like this, it would be advisable to get medically checked for a sexually transmitted illness/disease (STI/STD). If it comes back clear, then there is no need to inform the wife about it. However, if a sexually transmitted disease is found, then my humble view is that the wife should be made aware so that precaution may be taken. Otherwise, it would be unfair to the wife, especially if she found out later. It will also save you from feeling guilty later.
